The Emerging Role of Vitamin D in Sports Physical Therapy: A Review

ABSTRACT

Vitamin D, a secosteroid hormone, provides a plethora of physiological effects that impact athletic performance, injury prevention, and recovery. Vitamin D generates diverse musculoskeletal (bone and muscle) and extraskeletal (anti-inflammatory and antimicrobial) effects though gene activation. Sufficient stores of vitamin D (defined as 25-hydroxyvitamin D3 >30 ng/mL) are critical to optimize injury prevention and are obtained through sunlight exposure, diet, and supplementation. Vitamin D deficiency is not only associated with an increased susceptibility to stress fractures, muscular injury, and pain but it can also delay recovery post injury. Conversion from vitamin D deficient states to sufficiency provides maximal benefits and is important for physical therapists to recognize. Most athletes fail to meet the physiologic levels of vitamin D necessary to support optimal performance; emerging research indicates that vitamin D levels in the range of 40−60 ng/mL establish the physiologic environment for peak performance. These higher vitamin D concentrations are realistically obtained only with oral supplementation and have recently been linked to increases in speed and strength. Achieving vitamin D levels above this "optimal" threshold are not recommended. Recognition and correction of vitamin D deficiency is therefore recommended as a systematic approach to improve bone health, muscle function, physical performance, and recovery.
