ABSTRAKT

This article, which reviews pain therapy in postherpetic neuralgia, is based largely on the treatment of 2,300 Japanese patients with herpes zoster-related pain (80%) and postherpetic neuralgia (20%), with nerve block therapy. In the early stages of herpes zoster, acyclovir and Ara-A plus nerve block therapy were prescribed. Based on 20-years experience, we find that epidural infusion, given on a continuous basis, shortens the duration of pain, minimizes tactile allodynia, and diminishes postherpetic neuralgia. The location of affected primary sensory neurons and putative mechanisms of postherpetic neuralgia are discussed.
