Concepts in the Design of Artificial Ligaments

This paper focuses primarily on concepts in the design of artificial implants for the reconstruction of the anterior cruciate ligament (ACL). In order to address this issue effectively, the paper had to be in part reflective. Therefore, it includes a brief discussion of the reported experiences of ligament reconstructive procedures, those which have used (and still use) autogenous tissue, as well as those that have used various artificial implants. In the literature, there is a paucity of long-term follow-up results, from both user camps, and many of these results are concerned with artificial ligament implants. An analysis of failure of artificial implant reconstructions examines the causes and modes of failure. The complex interplay between these different factors was discussed, and on the basis of this reflective process, a holistic approach to the entire endeavor of ACL reconstruction appears most appropriate. In such an approach, all factors affecting the outcome of the surgery, which includes implicit expectations, must be taken into account.
