Spastic Paretic Gait: An Approach to Evaluation and Treatment

SINOPSIS

An approach to evaluation and treatment of spastic paretic gait is presented with a focus on the utilization of quantitative gait analysis in the formulation of a therapeutic plan. Presented is an overview of the typical spastic paretic gait patterns and an approach to their evaluation including strength, tone, range of motion, orthopedic deformity, observational gait evaluation, and quantitative gait evaluation. The procedures for gait analysis, including preparation and acquisition of the kinematic, kinetic, and dynamic EMG data, are presented. The approach to treatment utilizing standard physiatric interventions are discussed. Identifying each patient's unique set of impairments, disabilities, and compensations, regardless of underlying neurological disease, is critical for implementation of a focused and optimal therapeutic plan to maximize ambulatory function.
