Applying Theory to the Critical Review of Evidence from Music-Based Rehabilitation Research

ABSTRACT

Recent publications adopting a neurological perspective to music-based therapy signal a paradigmatic change. Three theories adopting a neurological perspective are used to critically review recent music−based therapy research. The three theories, rhythmic entrainment, emotional induction, and neural plasticity, are underpinned by neurological research. These theories can explain findings from multiple music-based therapies used to rehabilitate motor, cognitive, and emotional trauma. The theory of rhythmic entrainment provides an explanatory framework for therapies designed to treat patients with stroke, Parkinson's disease, traumatic brain injury, cerebral palsy, and other movement disorders. The theory of emotional induction explains the effect of neurotransmitters and the rehabilitation of depressed patients with stroke. It is also linked to rhythmic entrainment therapy. Neuroplasticity refers to the brain's ability to reorganize itself by forming new neural connections through mechanisms such as "axonal sprouting". The theory of neuroplasticity explains neural reorganization induced through music-based therapies and can be seen in the use of therapies to rehabilitate motor function and speech among patients with stroke. The review argues that an explanation of research findings may be further strengthened through the explicit inclusion of these three theoretical frameworks.
