Hemiplegic Shoulder Pain: Diagnosis and Management

Stroke is a significant source of mortality and long-term disability in the United States. Of those who survive a stroke, approximately half will have hemiplegia. Half of those with hemiplegia will live with a nonfunctional arm. Hemiplegic shoulder pain (HSP) occurs in the majority of patients with hemiplegia, and the presence of HSP reduces participation and worsens outcomes in rehabilitation. Despite its high prevalence, controversy exists regarding risk factors, etiology, diagnosis, and treatment of this condition. The causes of HSP are multifactorial (both peripheral and central), and the treatment of HSP is often multifaceted. The key to treatment of HSP is prevention. Proper positioning and range-of-motion exercises are the first line of treatment. This article will discuss the causes, identification, prevention, and treatment of HSP and includes a review of recent literature related to HSP.
