Repeatability and Reproducibility of Transcranial Doppler Ultrasound Measurements in Stroke: A Systematic Review

Cerebral blood flow regulation is considered a hallmark of brain health, in particular, for individuals who have survived a stroke. Transcranial Doppler (TCD) ultrasound is routinely used to assess cerebral blood velocity (CBv) during the acute stroke period to diagnose and locate occlusions of intracerebral blood vessels. However, resting TCD measurements and their within-day repeatability and between-day reproducibility have been less investigated in individuals who have suffered a chronic stroke. Repeated assessments of CBv measurements in stroke patients is essential and may enable us to better understand and refine neurorehabilitation protocols for post-stroke functional recovery. Therefore, the aim of this systematic review was to examine studies that have investigated the repeatability and reproducibility of TCD for assessment of CBv in individuals with stroke. We performed a systematic review of literature by searching PubMed, cumulative index of nursing and allied health literature (CINAHL), and excerpta medica database (EMBASE) for studies with stroke, CBv, and TCD reliability. From a total of 866 studies, only 2 studies were included in this review. Our review suggested that there is no clear evidence to support the repeatability and reproducibility of using TCD in stroke.
